What is the process and conditions to renew the formality?

I. For investment in general investment activities "Businesses not in the control account"
1. Application form for agricultural and forestry business license (with 02 3x4 photos) according to the application form of Department of Planning and Cooperation, Ministry of Agriculture and Forestry;
2. Summary of activities in the past year (according to the draft summary report of the Department of Planning and Cooperation, Agriculture and Forestry with confirmation from the Department of Agriculture and Forestry at the provincial level, the capital city and the related office of Agriculture and Forestry at the district level);
3. Enterprise registration certificate;
4. Tax registration certificate or tax liability certificate;
5. License to operate agriculture and forestry (last year).
Note: 1. In the event that the company has been active in several provinces, it must make a specific summary of each province through proper verification from the agriculture and forestry office at the district level and the agriculture and forestry department at the provincial level, the relevant capital city where the project activities have been carried out;
For activities related to services and general export-import trade services for distribution, there is no certification from the Agriculture and Forestry Office at the district level and the Agriculture and Forestry Department at the provincial level.
2. In case there is no summary report on the activities of the past year and there is no confirmation from the Office of Agriculture and Forestry at the district level, the Department of Agriculture and Forestry at the provincial level, the relevant capital. The Department of Planning and Cooperation of the Ministry of Agriculture and Forestry must cooperate with the Department, relevant local parties and companies to gather information, monitor and inspect the activities of the actual business, and create a joint record with proper verification to be used as a basis for presenting a report to the upper level in considering the license.
II. For investment in the control account

2. The summary report of the activities carried out in the past year (according to the draft summary report of the Department of Planning and Cooperation, Ministry of Agriculture and Forestry with confirmation from the Department of Agriculture and Forestry at the provincial level, the capital city and the related office of Agriculture and Forestry at the district level).
3. The land certificate and/or promotion agreement in form 2+3, which has been approved and certified must be complete (in case there are any amendments).
4. Investment license.
5. Enterprise registration certificate.
6. Tax registration certificate or tax liability certificate.
7. License to operate agriculture and forestry (last year).

2). In the case that the company does not have a summary report on its activities in the past year and there is no confirmation from the District Agriculture and Forestry Office, the Provincial Agriculture and Forestry Department, the relevant capital. The Department of Planning and Cooperation, the Ministry of Agriculture must coordinate with the Department, relevant local parties and companies to collect data, monitor and inspect the activities of the actual business and create a joint record with the correct certification to be used as a basis to present a report to the upper level in consideration of the license.
III. For investment in concession account activities
1. Application form for agricultural and forestry business license (with 02 3x4 photos) according to the application form of Department of Planning and Cooperation, Ministry of Agriculture and Forestry.
2. The summary report of the activities of the previous year (according to the draft summary report of the Department of Planning and Cooperation, Ministry of Agriculture and Forestry with the certification of the Department of Agriculture and Forestry at the provincial level, the capital city and the office of Agriculture and Forestry at the relevant district level).
3. The PDA project development agreement/concession agreement with the CA government, which has been approved and certified, must be complete (in case there are any amendments).
4. State Land Lease and Concession Contract Form 1+4 including land survey or land certificate and/or promotion contract in Form 2+3, which has been approved and certified to be complete (in case there are any amendments).
5. Environmental and Social Impact Assessment Report (ESIA) or Preliminary Environmental Impact Assessment Report (IEE); Or create an environmental management and monitoring plan (EMMP) or a social monitoring and management plan (SMMP), which has been approved and certified by the Ministry of Natural Resources and Environment (in case there are any amendments).
6. Investment license.
